WHY HAVE AN INDIVIDUAL PLAN?
An individual plan identifies areas essential for a person’s health and wellbeing. It allows the individual to be in control of their own life. Their are many ways to create an individual plan. Following is an example of what you might include in an individual plan.

1. The individual’s goals and strategies to achieve them.

2. Information for support workers and others involved in people with disabilities lives that empowers them to work on their goals and have a positive and productive life. Key areas in a plan include.

  ● Who the person is
  ● Their wellbeing, needs and dreams
  ● Necessary supports
  ● Their likes and dislikes
  ● Overcoming challenges
  ● Strengths and interests
  ● Independent life skills

3. Individuals are involved in ongoing review of their plan as needs and goals change over time.

THE EIGHT NDIS DOMAINS
The NDIS Domains cover all aspects of living and having a good life. Adam Support assists individuals to consider which of these domains are important to him/her during the planning process.

  ● Daily Living ● Health and Wellbeing
  ● Home ● Social and Community
  ● Choice and control ● Lifelong learning
  ● Work ● Relationships

An individual plan of this type is a useful way to prepare for the NDIS for when participants have NDIS planning meetings. NDIS planning is often defaults to a cost factor, whereas the individual plan provides information on what you actually need.
